WORKSHOP REGISTRATION

Please complete the attached registration form and mail it with your check to the address listed on the form by March 15, 1998. Checks should be made payable to "ORNL/PDE". Checks must be in U.S. dollars drawn on a U.S bank. After April 15, no refunds will be made. Requests for refunds must be made in writing and sent to Stephanie Burke at the address below.

The registration fee for this Joint Workshop is $90 US. The fee includes all meeting costs (audio/visual, meeting room fees), Wednesday through Friday continental breakfasts and breaks. It also includes a banquet (with no host bar) Thursday night, If attendees wish to bring a guest(s) to the banquet, it will cost an additional $24.00 per person. Please indicate on the registration form whether or not you will be bringing a guest(s), guest name(s), and include this additional charge with your registration fee.
